Irinotecan Hydrochloride falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Theaflavin interacts with.

In vitro, theaflavin inhibits organic anion-transporting polypeptide (OATP)2B1. OATP2B1 is expressed in the small intestine and liver and is responsible for the uptake of drugs and other compounds. This interaction has not been shown in humans. However, theaflavin might reduce the absorption of drugs that are OATP2B1 substrates. Use with caution.

Here's the gist with theaflavin (the antioxidant in black tea) and your irinotecan. In a test tube, theaflavin slowed down a transporter called OATP2B1 that helps move certain drugs into cells. Since irinotecan relies on transporters like this, the worry is that theaflavin could lower how much of the drug gets where it needs to go, which might make it work less well against the cancer.

I want to be straight with you, though: this was only seen in lab studies, never in actual people, so it's more of a theoretical flag than a proven problem. A normal cup of black tea is very unlikely to matter. Concentrated theaflavin extracts are the bigger question mark. Because irinotecan is serious chemo, please don't add any supplement without first checking with your oncology team or pharmacist.
