Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Elranatamab (Elrexfio) is a strong immune-based treatment for multiple myeloma. It works by revving up your own T-cells to hunt down cancer cells. Its whole job depends on how your immune system is behaving. That is where the concern with Trichopus zeylanicus comes in.

This herb has been studied mostly in animals and lab dishes, where it seemed to act as an immune stimulant. In theory, stirring up the immune system in unpredictable ways could clash with a therapy like elranatamab that is already carefully steering your immune response. Honestly, the evidence here is very thin, so I would not panic. But since this is a serious cancer drug, please don't add this supplement without first checking with your oncology team or pharmacist.

From our pharmacist’s overview on the Elranatamab page

“Elranatamab (Elrexfio) fights multiple myeloma by recruiting the immune system against the cancer, and that mechanism is exactly why I want people on it to read this page carefully rather than skim it. Most of the interactions here involve herbs and mushroom extracts that stimulate immune activity, which could in theory interfere with…”
