Theoretically, taking high doses of vitamin A in combination with other potentially hepatotoxic drugs might increase the risk of liver disease.
The tolerable upper intake level (UL) is the highest level of intake that is likely to pose no risk of adverse effects. Doses of vitamin A above the UL can cause hepatotoxicity, ranging from elevated liver enzymes to liver failure.

Here's the thing to keep in mind with baricitinib (Olumiant) and vitamin A. This isn't about one changing the levels of the other in your blood. Instead, it's that both can be tough on the liver. Baricitinib can sometimes raise liver enzymes, and that's why your doctor checks bloodwork while you're on it.

The worry is mostly theoretical and based on expert opinion, not hard studies. It really only applies to high vitamin A doses above the safe upper limit, not the small amounts in a normal diet or a standard multivitamin. Stacking large vitamin A doses on top of baricitinib could, in theory, add extra strain on the liver. Before adding any vitamin A supplement, check in with your pharmacist or doctor so they can keep an eye on your liver tests.

What Is Baricitinib Used For?

Brand name: Olumiant

Olumiant (baricitinib) tablets are approved for three conditions in adults. It treats moderately to severely active rheumatoid arthritis in people who haven't responded well enough to one or more TNF blocker medications. It also treats severe alopecia areata (significant hair loss from an autoimmune condition).

Olumiant is additionally approved to treat COVID-19 in hospitalized adults who need supplemental oxygen, mechanical ventilation, or ECMO (a machine that does the work of the heart and lungs). Each use has specific limitations — for example, Olumiant should not be combined with other JAK inhibitors or certain strong immunosuppressants for rheumatoid arthritis or alopecia areata.
