Adagrasib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Adagrasib (Krazati) is a targeted cancer pill used for certain lung cancers, and your liver clears it using an enzyme called CYP3A4. The thinking here is that piperine, the spicy compound in white pepper, may slow that same enzyme down. If it does, your body could clear adagrasib a little more slowly, letting levels creep up and possibly making side effects like nausea, diarrhea, fatigue, or changes in your heart's QT rhythm more likely.

Honestly though, this is a theoretical concern based on lab studies, not real patient cases. Normal cooking amounts of white pepper on your food are very unlikely to be a problem. Concentrated piperine supplements are where I'd be cautious. Since adagrasib is a serious cancer medicine, please run any pepper supplement past your oncologist or pharmacist first.

About Adagrasib

What Is Adagrasib Used For?

Brand name: Krazati

Krazati (adagrasib) is used to treat adults with locally advanced or metastatic non-small cell lung cancer (NSCLC) that carries a specific genetic change called a KRAS G12C mutation. It is used as a single agent for NSCLC after at least one prior treatment, and patients must be confirmed to have this mutation by an FDA-approved test.

Krazati is also used in combination with another drug called cetuximab to treat adults with KRAS G12C-mutated locally advanced or metastatic colorectal cancer (CRC) who have already been treated with specific prior chemotherapy regimens. Both uses were approved under the FDA's accelerated approval program based on tumor response data, and continued approval may depend on results from confirmatory trials.
