Alfentanil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the short version: alfentanil is a powerful opioid painkiller used during surgery and other procedures, and your body relies on an enzyme called CYP3A4 to break it down and clear it out. Piperine, the spicy compound in white pepper, has shown in lab studies that it can slow that same enzyme down. If CYP3A4 works more slowly, alfentanil could linger longer and hit harder, meaning more drowsiness, slowed breathing, low blood pressure, or deep sedation.

Now, take a breath. This is theoretical, based on test-tube findings, not real patient cases. The sprinkle of white pepper on your dinner is very unlikely to matter. Alfentanil is also given in monitored hospital settings, so if you take concentrated pepper supplements, just mention it to your doctor or pharmacist beforehand.
