Darunavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with Prezcobix (darunavir plus cobicistat). Darunavir is broken down by an enzyme in your body called CYP3A4. In test-tube studies, piperine (the spicy compound in white pepper) appears to slow that same enzyme down a little. If that happened in real life, darunavir could build up slightly, which in theory could mean more side effects like nausea, diarrhea, headache, or rash.

I want to keep this in perspective though. This is theoretical, based only on lab data, not on people. Cobicistat is already a strong booster, so a little spice on your dinner is very unlikely to matter. Concentrated piperine or pepper supplements are the bigger question mark. With an HIV regimen, it's always worth a quick chat with your pharmacist before adding any supplement.

What Is Darunavir and Cobicistat Used For?

Brand name: Prezcobix

Prezcobix and Prezcobix Ped are used to treat HIV-1 (human immunodeficiency virus type 1) infection. They are always used together with other antiretroviral (anti-HIV) medicines — never alone. Both products are approved for people who are new to HIV treatment (treatment-naïve) and for those who have taken HIV medicines before (treatment-experienced), as long as certain darunavir resistance mutations are not present.

Prezcobix tablets are approved for adults and pediatric patients weighing at least 40 kg. Prezcobix Ped tablets for oral suspension extend coverage to pediatric patients 3 years of age and older weighing at least 15 kg.
