Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Fruquintinib
Fruquintinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with white pepper and your Fruquintinib (Fruzaqla). This cancer medication is cleared from your body partly by a liver enzyme called CYP3A4. Piperine, the spicy compound in white pepper, has shown in lab studies that it can slow that same enzyme down. In theory, if the enzyme works slower, Fruquintinib could build up a bit more than expected, which might mean stronger side effects like high blood pressure, mouth sores, fatigue, hand-foot skin reactions, or digestive upset.
Now, the important part: this is theoretical, based only on test-tube research, not real patient data. Everyday amounts of pepper on your food are very unlikely to cause trouble. I'd only be cautious with concentrated piperine supplements. Since Fruquintinib is a serious drug, please run any pepper supplements by your oncology team first.

From our pharmacist’s overview on the Fruquintinib page
“Fruquintinib (Fruzaqla) is an oral targeted therapy for advanced colorectal cancer, which means keeping its blood level steady is part of what makes it work, and I would treat this supplement list with genuine care. Of the 168 ingredients on file, the large majority interact through CYP3A4, the liver enzyme that clears the drug, so the…”

About White Pepper
What is White Pepper?
- Digestive upset
- Seasoning and appetite support
- Cough and cold remedies (traditional)
- Minor aches and pains (traditional)
- Antimicrobial (traditional)
White pepper comes from the same plant as black pepper and is mostly used as a kitchen spice. In normal food amounts it is generally safe for most people, but there is little solid human research supporting its use as a medicine. Talk with your pharmacist or doctor before using concentrated supplements, especially if you are pregnant, breastfeeding, or have a health condition.
About Fruquintinib
What Is Fruquintinib Used For?
Brand name: Fruzaqla
Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.
Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
