Polatuzumab Vedotin-Piiq falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Polivy and white pepper. Polivy is an antibody-drug conjugate, and the cancer-killing piece it carries (called MMAE) gets broken down partly by a liver enzyme named CYP3A4. The piperine in white pepper has, in lab dishes, shown a mild ability to slow that same enzyme. The idea is that if CYP3A4 is slowed, a bit more of that toxic payload could linger, which in theory might bump up Polivy's side effects like low blood counts, infections, nerve tingling or numbness in your hands and feet, fatigue, and nausea.
Now, the important part: this is purely theoretical and based on test-tube work, not real patients. Normal cooking amounts of white pepper are very unlikely to matter. Concentrated piperine supplements are a different story, so please run those by your oncology team before starting them.

Polatuzumab vedotin-piiq injection is used along with bendamustine (Belrapzo, Treanda) and rituximab (Rituxan) in adults to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not improve or improved but returned after treatment with at least two other chemotherapy medications. Polatuzumab vedotin-piiq is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
