The evidence for L-threonine as a supplement is thin. For amyotrophic lateral sclerosis (ALS, Lou Gehrig's disease), it appears possibly ineffective.
For multiple sclerosis, familial spastic paraparesis, and spasticity, we don't have enough reliable evidence to say whether it works or not.

Threonine from food is safe at normal dietary amounts. When you take it as a supplement at higher doses, though, safety hasn't been well studied.
