What’s inside
Full disclosure
This product contains one active ingredient: acetyl-L-carnitine 500 mg per capsule. Acetyl-L-carnitine is a form of carnitine, a compound your body makes naturally and uses to help cells generate energy.
It's included in this formula because research suggests it may play a role in brain function and energy metabolism. The capsule also holds inactive ingredients—hypromellose, microcrystalline cellulose, leucine, and silicon dioxide—which serve as binders, fillers, and flow agents to hold the capsule together.
Does it work?
Moderate evidence
Acetyl-L-carnitine carries a rating of possibly effective for several conditions based on the evidence we hold: age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic nerve pain (neuropathy), and depression. "Possibly effective" means there's some research support, but the evidence isn't yet strong enough to call it proven. If you're considering this supplement for one of these purposes, it's worth discussing with your doctor or pharmacist whether the current evidence makes sense for your situation.
How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated when taken short-term, but long-term safety data isn't fully established, so we can't say for certain how it performs over months or years. Common side effects include agitation, dry mouth, headache, trouble sleeping, and reduced appetite.
A few people have reported a fishy odor in urine, breath, or sweat. Gastrointestinal upset—nausea, vomiting, diarrhea, constipation, or abdominal pain—can occur, though it doesn't happen more often than with placebo in most studies.
During pregnancy, there isn't enough reliable safety information, so it's best avoided unless your doctor advises otherwise. The same caution applies to breastfeeding: safety is unknown, so avoiding it is the safer choice.
If you're pregnant, planning to become pregnant, or breastfeeding, talk with your doctor or pharmacist before starting.
Meds to double-check
Moderate interaction found
Before you start this product, check with your pharmacist if you take any serotonergic drugs (antidepressants, some migraine or pain medications), blood thinners like warfarin or acenocoumarol, or thyroid hormone replacement. These are the medication types documented to interact with acetyl-L-carnitine.
Use the medication checker on this page to look up your specific prescriptions.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Acetyl-L-carnitine might be worth considering if you're interested in cognitive support or have one of the conditions it's possibly effective for, but you'll want to rule out interactions with blood thinners, serotonergic drugs, and thyroid medication first. If you take any prescription medications, check them here before you begin.
Talk to your pharmacist or doctor about your goals and whether the current evidence fits your needs.

Acetyl-L-Carnitine 500 mg by Thorne Drug Interactions
HelloPharmacist Interaction Report
Acetyl-L-Carnitine 500 mg by Thorne can interact with several medication types.
The most serious concern is with serotonergic drugs—antidepressants, migraine medications, and similar drugs that work on serotonin. Animal research shows acetyl-L-carnitine can raise serotonin levels in the brain, which might raise the risk of serotonin syndrome (a dangerous side effect marked by agitation, confusion, rapid heartbeat, and high body temperature) or blood vessel tightening in the brain.
Read the full breakdown — every affected drug type, severity by severity
It also interacts with blood thinners. Acetyl-L-carnitine might strengthen the effect of warfarin (Coumadin) and acenocoumarol (Sintrom), which could raise your bleeding risk.
Additionally, this ingredient may reduce how well thyroid hormone replacement works by blocking the hormone's entry into cells—something you'd want to monitor if you take levothyroxine or another thyroid medication.
Although we hold data on these interactions, this ingredient was the only one we could fully check. Altogether, these interactions span 203 individual medications.
Use the medication checker on this page to look up your exact prescriptions before you start.

For each ingredient in Acetyl-L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Acetyl-L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
