Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: acetyl L-carnitine at 500 mg per capsule. Acetyl L-carnitine is a compound your body makes naturally and uses in energy production—it helps move fatty acids into cells so they can be burned for fuel.
The inactive ingredients are the capsule itself, silica, cellulose, and magnesium stearate (a flow agent).
Does it work?
Moderate evidence
Acetyl L-carnitine is rated possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy (nerve pain from diabetes), and depression, based on available evidence. That means there's some research support, but more study is needed to confirm how well it actually works for these uses.
The evidence we have isn't strong enough to call it definitely effective.
How safe is it?
Well-documented data
Acetyl L-carnitine is generally well tolerated for short-term use, but long-term safety hasn't been fully studied. The most common side effects are agitation, dry mouth, headache, insomnia, and reduced appetite.
Some people report a fishy odor in urine, breath, or sweat—that's from a metabolite of the supplement. Less commonly, it may cause nausea, vomiting, diarrhea, constipation, or stomach upset, though these don't happen more often than with a placebo in most studies.
There isn't enough reliable safety information for pregnancy, so it's best avoided during pregnancy unless your doctor advises otherwise. Safety while breastfeeding is unknown—avoiding it is the safer choice.
If you're pregnant, planning pregnancy, or breastfeeding, talk with your doctor or pharmacist before using this supplement.
Meds to double-check
Moderate interaction found
Before you start this supplement, double-check with your pharmacist if you take serotonergic drugs (antidepressants, certain pain or migraine medications), blood thinners like warfarin or acenocoumarol, or thyroid hormone replacement. These are the medication types with documented moderate-severity interactions with acetyl L-carnitine.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Acetyl L-carnitine may be worth considering if you're interested in support for cognitive function, Alzheimer disease, nerve pain from diabetes, or mood—and your doctor agrees it's right for you. But if you take serotonergic drugs (like antidepressants), blood thinners, or thyroid hormone, you need to check with us first.
Talk to your pharmacist before starting, especially if you're taking any prescriptions.

Formulation
Supports brain health & energy+ Acetyl-L-Carnitine (ALC) is a molecule composed of acetic acid and L-carnitine bound together. It is produced naturally in humans with the greatest amounts being found in muscles, the brain and testicles. ALC is important in the production and release of one of the brain’s vital neurotransmitters, known as acetyl-choline. ALC has been studied for supporting brain health, and may also support the healthy function of the male reproductive system and intracellular energy transfer.

Acetyl L-Carnitine 500 mg by Woodstock Vitamins Drug Interactions
HelloPharmacist Interaction Report
Acetyl L-Carnitine 500 mg by Woodstock Vitamins has documented interactions with several medication types.
The most serious concern is with serotonergic drugs (antidepressants, certain pain medications, migraine treatments, and others that affect serotonin). Acetyl L-carnitine may increase serotonin levels in the brain, which could raise the risk of serotonin syndrome—a potentially serious condition with agitation, confusion, rapid heart rate, and high blood pressure.
Read the full breakdown — every affected drug type, severity by severity
You'll also want to check if you take blood thinners. Acetyl L-carnitine is theoretically thought to boost the anticoagulant effects of warfarin (Coumadin) and acenocoumarol (Sintrom), meaning your bleeding risk could increase.
The same concern applies to thyroid hormone replacement medication—acetyl L-carnitine may reduce how well it works by blocking thyroid hormone from entering cells.
Altogether, these interactions span 203 individual medications. Because these are theoretical risks based on how the ingredient works, not all people will experience a problem, but your exact medications need to be checked before you start.
Use the interaction checker on this page with your full medication list.

For each ingredient in Acetyl L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Acetyl L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
