Lactobacillus acidophilus is generally well tolerated in healthy adults and children. The product may cause mild gastrointestinal side effects — such as stomach discomfort, bloating, gas, or diarrhea — but these are uncommon.

Because this product contains live organisms, there is a theoretical concern that in rare cases they could cause infection in certain patients, particularly those with severely weakened immune systems; most documented cases have occurred in people already at high risk. Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
