Oral use carries a real risk of liver damage, including a condition called veno-occlusive disease, which can cause stomach pain, vomiting, diarrhea, fluid buildup in the belly, and liver enlargement within weeks of repeated exposure.
The safety data advises against use during pregnancy and while breastfeeding due to these alkaloid toxins and lack of safety information. Topically, absorption of these alkaloids is also a concern.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.
