Aloe vera products are generally well tolerated at normal doses, but oral aloe — especially the latex from the leaf's outer layer — carries real risks. It can cause abdominal pain, cramps, and diarrhea, and long-term use or high doses can lead to serious potassium loss (hypokalemia), which may trigger muscle weakness, irregular heartbeats, and other complications.

One documented case involved severe potassium loss in a patient taking a liter of aloe daily for two weeks. For pregnancy, aloe is rated possibly unsafe because the latex can stimulate the uterus and act as a strong laxative — don't use it by mouth during pregnancy.

Avoid it while breastfeeding too, because of limited safety data and laxative effects. Marshmallow and slippery elm have insufficient safety data for pregnancy and breastfeeding, so there isn't enough information to know either way — talk with your doctor or pharmacist for personalized advice if you're pregnant or nursing and considering this product.

Overuse of aloe latex can increase the risk of adverse effects from cardiac glycoside drugs, such as digoxin, due to potassium depletion. Overuse of aloe, along with cardiac glycoside drugs, can increase the risk of toxicity.
