Andrographis is generally well tolerated over the short term, but long-term safety and product quality aren't well established. Most common side effects—abdominal discomfort, diarrhea, nausea, dizziness, headache, fatigue, and rash—tend to happen at higher doses (5–10 mg/kg of andrographolide, the active component) or beyond 14 days of use.

Skin reactions including rash, itching, and hives are frequently reported. Rare serious effects include severe allergic reactions up to anaphylaxis.

Do not use during pregnancy because of theoretical and animal-study concerns about reproductive effects. Don't use while breastfeeding—there isn't enough safety information.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
