Andrographis is generally well tolerated in the short term, but the long-term safety and quality of products aren't well established yet. Common side effects include stomach discomfort, changes in taste, diarrhea, dizziness, fatigue, headache, nausea, vomiting, itching, and rash.

Higher doses (5–10 mg/kg of andrographolide or more) and use beyond 14 days make side effects more likely. Rare but serious allergic reactions, including anaphylaxis, have been reported.

Skin reactions — including severe ones like Stevens-Johnson syndrome — have also been reported, though they're uncommon. Avoid this product during pregnancy because of theoretical and animal-study concerns about reproductive effects.

There isn't enough safety information for breastfeeding, so avoid use then too. If you're pregnant, planning to become pregnant, or breastfeeding, talk with your doctor or pharmacist before considering this supplement.

Theoretically, andrographis might decrease the absorption of etoricoxib, although the clinical significance is unclear.
Animal research shows that andrographis extract, or the constituent andrographolide, taken orally with etoricoxib decreases the bioavailability of etoricoxib. However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
