HelloPharmacist Scorecard of Bael Alcohol Free by Hawaii Pharm
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Bael Alcohol Free contains one active ingredient: bael fruit extract (dry form). The extract comes from the bael tree, traditionally used in Ayurvedic and Thai medicine.
The product is rounded out with glycerin and water as inactive ingredients.
Does it work?
Couldn't assess
The evidence we hold rates bael's effectiveness for asthma and shigellosis (a bacterial infection causing diarrhea) as insufficient — meaning there isn't enough reliable research to say whether it works for either condition. If you're considering this product for one of those uses or another, talk with your doctor or pharmacist about what the current evidence actually supports.
How safe is it?
Well-documented data
The ripe bael fruit is eaten as food and is widely considered safe in that form, but concentrated supplements like this one lack strong safety data. Large amounts may cause stomach upset and constipation.
Bael is best avoided in pregnancy — some traditional sources warn against medicinal use at that time, and safety data is limited. Medicinal use should also be avoided while breastfeeding, as there isn't enough reliable information to know it's safe.
Meds to double-check
Moderate interaction found
Before taking this product, check with your doctor or pharmacist if you take antidiabetes drugs (like glimepiride), cholinergic medications (like donepezil or bethanechol), or drugs broken down by the liver enzymes CYP1A2 or CYP3A4 (such as amitriptyline, haloperidol, lovastatin, or ketoconazole). Blood glucose monitoring may be needed, and your doses might need adjustment.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Bael extract may have a role for some people, but it carries moderate interactions with blood sugar and brain-chemical medications, and the safety data for supplements is thin. If you take antidiabetes drugs or any cholinergic medication, you'll want to discuss this product with your doctor or pharmacist before starting — dose adjustments might be needed, and you'd need closer monitoring.
Pregnant people and those breastfeeding should avoid it.

Bael Alcohol Free by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Bael Alcohol Free by Hawaii Pharm contains bael fruit extract, which interacts with several medication types.
The most serious concern is with antidiabetes drugs (blood sugar medications) like glimepiride — bael may lower blood glucose levels on its own, and combining it with these drugs could push your blood sugar too low (hypoglycemia). If you take any antidiabetes medication, you'll need close blood glucose monitoring and possibly a dose adjustment.
Read the full breakdown — every affected drug type, severity by severity
Bael also has moderate interactions with cholinergic drugs (medications that work through acetylcholine, a brain chemical) — examples include donepezil for Alzheimer's disease and bethanechol for bladder issues. The extract may add to their effects and cause cholinergic side effects.
Additionally, bael may inhibit how your body breaks down certain medications through two liver enzymes (CYP1A2 and CYP3A4), theoretically raising levels of drugs like amitriptyline, haloperidol, lovastatin, and ketoconazole — though this hasn't been confirmed in humans yet. Altogether, these interactions span 820 individual medications.
Before starting this product, check your exact medications with your doctor or pharmacist.

For each ingredient in Bael Alcohol Free with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Bael Fruit Extract, Dry
Antidiabetes Drugs
Evidence from animal research suggests that extracts of bael seed and leaf can reduce blood glucose levels. Theoretically, bael might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.
Cholinergic Drugs
Bael leaf extract shows acetylcholinesterase (AChE) inhibitory activity in vitro. Theoretically, bael might have additive effects with cholinergic drugs and increase the risk of cholinergic side effects.
Cholinergic drugs include bethanechol (Urecholine), donepezil (Aricept), echothiophate (Phospholine Iodide), edrophonium (Enlon, Reversol, Tensilon), neostigmine (Prostigmin), physostigmine (Antilirium), pyridostigmine (Mestinon, Regonol), succinylcholine (Anectine, Quelicin), and tacrine (Cognex).
Cytochrome P450 1A2 (Cyp1A2) Substrates
Bael extract and its constituent marmesinin inhibited cytochrome P450 1A2 (CYP1A2) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, bael might increase levels of drugs metabolized by CYP1A2.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others. Use bael cautiously or avoid in patients taking these drugs.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Bael and its constituents marmelosin and marmesinin inhibited cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, bael might increase levels of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use bael cautiously or avoid in patients taking these drugs.
