Thiamine is generally very safe by mouth at normal doses; allergic skin reactions are rare. Benfotiamine is generally well tolerated in studies, though long-term safety isn't fully known.

Benfotiamine is a lipid-soluble derivative of vitamin B1 (thiamin) with high bioavailability. It is better absorbed than water-soluble forms of thiamin and has an estimated bioavailability of up to 3.6 times as high as compared to thiamin hydrochloride, making benfotiamine especially useful for people with absorption difficulties.

Trimethoprim might increase blood levels of thiamine.
In vitro, animal, and clinical research suggest that trimethoprim inhibits intestinal thiamine transporter ThTR-2, hepatic transporter OCT1, and renal transporters OCT2, MATE1, and MATE2, resulting in paradoxically increased thiamine plasma concentrations.
