Bladdex contains four active ingredients. Marshmallow is an herb traditionally used to soothe mucous membranes.
Couch grass and juniper berry are botanical diuretics—they're meant to support urinary function. Oregon grape, also called barberry, contains berberine, an alkaloid with anti-inflammatory properties.
The product also includes vegetable glycerin and purified water as inactive ingredients to deliver the blend in liquid form.

The evidence for these ingredients is not yet established in our data. Marshmallow, couch grass, and juniper all show insufficient reliable evidence for their traditional uses—urinary tract health, kidney stones, and related conditions.
Oregon grape is rated as possibly effective for psoriasis when used topically, but the other conditions studied (eczema, GERD, peptic ulcers) lack sufficient evidence. Since this is a liquid formula meant for oral use, the topical psoriasis data doesn't apply here.
How safe is it?
Well-documented data
Marshmallow, couch grass, and juniper are generally well tolerated in the short term, but human safety data are limited for all three. Oregon grape should be approached carefully with oral use; topical application is better studied.
Adverse effects reported for juniper include allergies and skin irritation (though this is primarily topical), and juniper should not be used long-term because medicinal doses may stress the kidneys. Oregon grape may cause itching, burning, or skin irritation topically.
All four ingredients carry pregnancy warnings: marshmallow, couch grass, and juniper lack enough safety data and should be avoided; Oregon grape (berberine) is traditionally contraindicated and should not be used. For breastfeeding: marshmallow and couch grass lack sufficient data; Oregon grape and berberine may pass to infants and should be avoided; juniper also lacks enough safety information and should be avoided.

The most serious concern is Oregon grape, which can increase the effects of drugs broken down by your liver (your body's metabolic enzymes), potentially raising their levels in your blood to unsafe amounts. This applies to a large number of common medications—including some heart drugs, blood pressure medications, and others metabolized by CYP2D6, CYP2C9, and CYP3A4 enzymes.
Oregon grape can also increase bleeding risk with blood thinners (anticoagulants) or antiplatelet drugs, lower blood sugar with diabetes medications, and boost the sedative effects of sleep aids or anti-anxiety drugs (CNS depressants).

Marshallow, another ingredient here, may reduce how well your body excretes lithium (a mood stabilizer), raising lithium levels and risking toxicity; it can also interfere with how your gut absorbs oral medications. Juniper berry may cause extra low blood sugar if you take diabetes drugs, reduce lithium excretion like marshmallow does, and amplify side effects from diuretic drugs (water pills).

Marshmallow
Lithium
Theoretically, due to potential diuretic effects, marshmallow might reduce excretion and increase levels of lithium.
Marshmallow is thought to have diuretic properties. To avoid lithium toxicity, the dose of lithium might need to be decreased when used with marshmallow.
Anticoagulant/Antiplatelet Drugs
Theoretically, marshmallow flower might have antiplatelet effects.
Animal research suggests that marshmallow flower extract has antiplatelet effects. However, the root and leaf of marshmallow, not the flower, are the plant parts most commonly found in dietary supplements. Theoretically, use of marshmallow flower with anticoagulant/antiplatelet drugs can have additive effects, and might increase the risk for bleeding in some patients.
Oral Drugs
Theoretically, mucilage in marshmallow might impair absorption of oral drugs.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
Oregon Grape
Anticoagulant/Antiplatelet Drugs
Theoretically, Oregon grape might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro and in vivo research suggests that berberine, a constituent of Oregon grape, can inhibit platelet aggregation.
Antidiabetes Drugs
Theoretically, Oregon grape might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Clinical research suggests that berberine, a constituent of Oregon grape, can lower blood glucose levels.
Antihypertensive Drugs
Theoretically, Oregon grape might increase the risk of hypotension when taken with antihypertensive drugs.
Animal research suggests that berberine, a constituent of Oregon grape, can have hypotensive effects. Also, an analysis of clinical evidence suggests that taking berberine in combination with amlodipine (Norvasc) can lower systolic and diastolic blood pressure when compared with taking amlodipine alone.
Cns Depressants
Theoretically, Oregon grape might increase the sedative effects of CNS depressants.
Animal research suggests that berberine, a constituent of Oregon grape, can have sedative effects.
Cyclosporine (Neoral, Sandimmune)
Theoretically, Oregon grape might increase the effects and adverse effects of cyclosporine.
Berberine, a constituent of Oregon grape, can reduce metabolism of cyclosporine and increase serum levels. It might inhibit cytochrome P450 3A4 (CYP3A4), which metabolizes cyclosporine.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, Oregon grape might increase serum levels of drugs metabolized by CYP2C9.
Preliminary clinical evidence suggests that berberine, a constituent of Oregon grape, can inhibit cytochrome P450 2C9 (CYP2C9).
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, Oregon grape might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, can inhibit cytochrome P450 2D6 (CYP2D6).
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, Oregon grape might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, moderately inhibits cytochrome P450 3A4 (CYP3A4).
P-Glycoprotein Substrates
Theoretically, Oregon grape might increase serum levels of drugs that are P-glycoprotein (P-gp) substrates.
In vitro research suggests that Oregon grape extracts inhibit P-gp efflux.
Juniper
Antidiabetes Drugs
Theoretically, taking juniper berry with antidiabetes medications might cause additive hypoglycemia.
Animal research shows that juniper berry can lower blood glucose.
Diuretic Drugs
Theoretically, juniper berry might increase the risk of adverse effects from diuretic drugs.
Juniper berry is thought to have mild diuretic effects.
Lithium
Theoretically, juniper berry might reduce lithium excretion and increase serum levels of lithium.
Juniper berry is thought to have mild diuretic effects.
