Does it work?
Couldn't assess
We don't have effectiveness data on file for blue cohosh in this product. The safety concerns with this ingredient are significant enough that effectiveness information alone won't tell you whether it's right for you—that's a conversation for your doctor or pharmacist.
How safe is it?
Well-documented data
Blue cohosh carries serious safety cautions. It contains compounds that affect the heart and has been linked to significant adverse events, including chest pain (angina), high blood pressure, increased heart rate, stomach upset, diarrhea, and cramping.
Most critically, it is not safe in pregnancy—it can stimulate the uterus and has been linked to severe harm to newborns, including heart attacks, heart failure, and shock. There isn't enough safety information to use it while breastfeeding either.
Meds to double-check
Moderate interaction found
Before taking this product, double-check with your doctor or pharmacist if you take blood pressure medications (such as captopril, enalapril, losartan, or valsartan), diabetes medications (including insulin, glimepiride, or glyburide), or use any nicotine products. These represent Moderate-severity interactions that could affect how your medications work.

Blue Cohosh by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Blue Cohosh by Hawaii Pharm has documented interactions with multiple medication types, and we need to be straightforward about the concerns here.
The most serious issue is with blood pressure medications (antihypertensives): blue cohosh can raise blood pressure by tightening blood vessels in the heart, which may make drugs like captopril, enalapril, losartan, and valsartan less effective. This is rated Moderate severity.
Read the full breakdown — every affected drug type, severity by severity
There's also a Moderate concern with diabetes medications. Blue cohosh may increase blood glucose levels, potentially reducing how well medicines like insulin, glimepiride, glyburide, pioglitazone, and rosiglitazone work for you.
Additionally, blue cohosh can amplify the effects of nicotine (Moderate severity)—something to know if you use nicotine products.
Altogether, these interactions span 259 individual medications. If you take any blood pressure medication, diabetes drug, or use nicotine in any form, check your exact medications with the search tool on this page before starting this product.
