What’s inside
Full disclosure
Blue Cohosh by Herb Pharm contains one active ingredient: blue cohosh rhizome with rootlet extract (Caulophyllum thalictroides). It comes as a liquid, which means the plant material is extracted into a base of cane alcohol and distilled water for easier absorption.
Does it work?
Couldn't assess
We hold no effectiveness ratings for blue cohosh in our data, so the evidence for what it may do isn't established in the information available to us. If you're considering it for a specific purpose, ask your doctor or pharmacist what the research actually shows for your situation.
How safe is it?
Well-documented data
Blue cohosh comes with serious safety warnings. The herb contains compounds that affect your heart, and it can cause chest pain (angina), high blood pressure, raised blood sugar, stomach upset, and nausea.
More concerning: using blue cohosh during pregnancy or shortly before delivery has been linked to heart attacks, heart failure, and shock in newborns — it's considered unsafe in pregnancy. There isn't enough safety information to recommend it while breastfeeding either.
If you're pregnant, planning pregnancy, or breastfeeding, do not use this product. Even outside of pregnancy, the heart effects are real enough that caution is warranted, especially if you have any heart or blood pressure history.
Meds to double-check

Blue Cohosh by Herb Pharm Drug Interactions
HelloPharmacist Interaction Report
Blue Cohosh by Herb Pharm has documented interactions with blood pressure medications, diabetes medications, and nicotine — and these are concerns worth taking seriously.
Read the full breakdown — every affected drug type, severity by severity
The most serious issue is with blood pressure drugs (Moderate severity). Blue cohosh contains compounds that can tighten blood vessels in the heart, which may raise your blood pressure and reduce how well your blood pressure medications work.
This includes ACE inhibitors like enalapril (Vasotec) and captopril (Capoten), as well as ARBs like losartan (Cozaar) and valsartan (Diovan), and calcium channel blockers like diltiazem. If you take any of these, use blue cohosh only under close supervision.
Blue cohosh may also raise blood sugar levels (Moderate severity), potentially making diabetes medications less effective. This includes insulin, sulfonylureas like glimepiride (Amaryl) and glyburide (DiaBeta), and thiazolidinediones like pioglitazone (Actos).
Additionally, blue cohosh can increase the effects of nicotine, which matters if you're using nicotine patches, gum, or other nicotine products.
Altogether, these interactions span 259 individual medications. Before you start blue cohosh, run your exact medications through the checker on this page — or better yet, talk it over with your doctor or pharmacist first.
