Bendroflumethiazide falls under ANTIHYPERTENSIVE DRUGS — the group Blue Cohosh interacts with.

Constituents in blue cohosh might increase blood pressure by causing coronary vasoconstriction. Theoretically, concomitant use might decrease the effectiveness of drugs used for angina and high blood pressure; use with caution. Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), Amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.

Bendroflumethiazide is a water pill (thiazide diuretic) that helps lower your blood pressure. Here's the concern with blue cohosh: some of the natural compounds in this herb may tighten blood vessels and nudge blood pressure up. If that happens, it could work against what your bendroflumethiazide is trying to do, making it harder to keep your numbers in a healthy range.

I want to be honest that this is a theoretical worry based on how blue cohosh behaves in the lab, not on solid studies in people taking this exact medicine. Still, blue cohosh has real safety concerns of its own, so it isn't an herb I'd recommend casually. Please check with your pharmacist or doctor before pairing the two, and keep an eye on your blood pressure if you do.
