Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Blue Cohosh interacts with.

Constituents in blue cohosh might increase blood pressure by causing coronary vasoconstriction. Theoretically, concomitant use might decrease the effectiveness of drugs used for angina and high blood pressure; use with caution. Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), Amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.

Here's the simple version: iloprost works by relaxing and widening your blood vessels, especially the ones in your lungs, to ease pulmonary arterial hypertension. Blue cohosh may do the opposite. Some of its plant compounds are thought to tighten blood vessels and nudge blood pressure up. If that happens, it could work against what iloprost is trying to do and blunt some of its benefit, possibly leaving you with more shortness of breath, tiredness, or dizziness.

I want to be honest about the evidence though, this concern is theoretical, based on how the herb behaves in the lab, not on real cases in people taking iloprost. Still, with a serious lung condition like yours, this isn't an herb to experiment with. Please check with your doctor or pharmacist before adding blue cohosh.

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
