What’s inside
Full disclosure
This product contains one active ingredient: blue flag root extract (dry form). Blue flag is a plant whose root has been used traditionally, though we don't have established effectiveness ratings in our data for it.
The liquid also contains two inactive ingredients—glycerin and water—as carriers.
Does it work?
Couldn't assess
We don't hold effectiveness ratings for blue flag root extract, so evidence for how well it works isn't established in the data we have on file.
How safe is it?
Well-documented data
Blue flag can cause nausea, vomiting, and irritation—especially the fresh root irritates the lining of the mouth and throat. The plant's volatile oil can also trigger headache, eye inflammation, and throat irritation.
The safety data advises against use during pregnancy due to potential toxicity and lack of safety information. It's also advised against while breastfeeding because of insufficient safety data.
Meds to double-check
Major interaction found
Check with your pharmacist if you take digoxin (a heart medication)—overuse of blue flag raises the theoretical risk of serious effects. Also flag diuretics (water pills) because blue flag might worsen potassium loss, and warfarin or other blood thinners because blue flag's laxative effects can increase bleeding risk.
The bottom line

Blue Flag Alcohol Free by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Blue Flag Alcohol Free by Hawaii Pharm contains one ingredient—blue flag root extract—that interacts with a notable number of medications.
The most serious concern is with digoxin (Lanoxin), a heart medication: overuse of this product theoretically increases the risk of adverse effects from this drug.
Read the full breakdown — every affected drug type, severity by severity
Moderate interactions exist with two major drug categories. Diuretics (water pills) are a concern because blue flag might compound the potassium loss these drugs cause, raising the risk of low potassium (hypokalemia); some patients may need potassium supplementation adjusted.
Warfarin (Coumadin), a blood thinner, can be affected because blue flag has laxative effects and can trigger diarrhea, which increases warfarin's blood-thinning action and bleeding risk.
Altogether, these interactions span 78 individual medications. Before you start blue flag, use the search tool below to check your exact prescriptions—especially if you take heart or blood pressure medications, diuretics, or blood thinners.

For each ingredient in Blue Flag Alcohol Free with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Blue Flag Root Extract, Dry
Digoxin (Lanoxin)
Theoretically, overuse or abuse of this product increases the risk of adverse effects from cardiac glycoside drugs.
Diuretic Drugs
Overuse of blue flag might compound diuretic-induced potassium loss. There is some concern that people taking blue flag along with potassium depleting diuretics might have an increased risk for hypokalemia. Initiation of potassium supplementation or an increase in potassium supplement dose may be necessary for some patients. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.
Warfarin (Coumadin)
Blue flag has stimulant laxative effects. In some people blue flag can cause diarrhea. Diarrhea can increase the effects of warfarin, increase international normalized ratio (INR), and increase the risk of bleeding. Advise patients who take warfarin not to take excessive amounts of blue flag.
