HelloPharmacist Scorecard of Carnitine 500 by GNC Mega Men
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
GNC Mega Men Carnitine 500 is a single-ingredient supplement containing 500 mg of L-carnitine per capsule. L-carnitine is a compound your body uses to move fatty acids into cells where they're burned for energy.
The capsule itself is made from gelatin.
Does it work?
Insufficient evidence
The evidence for L-carnitine is mixed. It's rated effective for treating L-carnitine deficiency — a real condition that causes muscle weakness and fatigue.
For other uses, the data we hold shows it's possibly effective for high cholesterol, congestive heart failure, and angina. If you're taking it for one of those heart or cholesterol issues, talk with your doctor about whether the evidence supports it for your situation.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses, but higher amounts can cause stomach upset, nausea, diarrhea, heartburn, and a fishy body odor. A small number of people have reported abdominal cramps and reduced appetite.
Seizures are rare but have been documented. The product carries a caution that it's not well studied and should be used only under medical supervision.
Safety while breastfeeding is not well studied, so caution is advised — talk with your doctor or pharmacist if you're nursing.
Meds to double-check
Moderate interaction found
Before taking this product, double-check with your pharmacist if you're on blood thinners (warfarin, acenocoumarol, or other anticoagulants) — L-carnitine may strengthen their effect and raise bleeding risk. Also confirm if you take thyroid hormone replacement, since L-carnitine may reduce its effectiveness.
The bottom line
Scorecard at a glanceFully disclosed formula with insufficient evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This supplement works for L-carnitine deficiency, and there's possibly helpful evidence for heart and cholesterol issues, but it's not a stand-in for prescription treatment. If you take blood thinners or thyroid medication, check with your pharmacist before starting — L-carnitine can interfere with both.
Side effects are usually mild, but watch for stomach upset.

Carnitine 500 by GNC Mega Men Drug Interactions
HelloPharmacist Interaction Report
GNC Mega Men Carnitine 500 contains L-carnitine, which does interact with blood thinners and thyroid medications.
The most serious concern is with blood thinners (anticoagulants) like warfarin (Coumadin) and acenocoumarol (Sintrom) — L-carnitine may increase their blood-thinning effects, raising your risk of bleeding. There are case reports of INR elevation (a measure of how thin your blood is) when L-carnitine was used with acenocoumarol.
Read the full breakdown — every affected drug type, severity by severity
L-carnitine also theoretically decreases how well thyroid hormone replacement works by blocking the hormone from entering your cells. If you take thyroid medication, your dose effectiveness could drop.
Altogether, these interactions span 19 individual medications. Before you start this supplement, use the medication checker on this page to confirm your exact drugs are clear.

For each ingredient in Carnitine 500 with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
