HelloPharmacist Scorecard of Cascara Sagrada 270 mg by Nature's Way
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: Cascara Sagrada at 270 mg per capsule. It's a stimulant laxative derived from the bark of a buckthorn tree, used to promote bowel movements.
The capsule itself is made from hypromellose (a plant-based film), cellulose, and silica — these are inactive ingredients that form and stabilize the capsule.
Does it work?
Not established
The evidence for cascara sagrada is limited. For bowel preparation, it's rated possibly ineffective — meaning the data don't support it for that use.
For general constipation, the evidence is insufficient to say whether it works or not. The fact is, we don't have strong trial data behind this supplement for either purpose.
How safe is it?
Well-documented data
When used short-term and as directed, cascara is generally well tolerated. The most common side effect is mild abdominal cramping and discomfort.
The real caution is long-term use — cascara can cause serious problems over time, including liver damage (rare), and with prolonged use you can develop protein in your urine, blood in your urine, and severe weight loss and weakness. Fresh or improperly aged cascara bark can cause severe vomiting.
During pregnancy, stimulant laxatives like cascara are generally not recommended. While breastfeeding, the active compounds may pass into breast milk, so it's best avoided.
If you're pregnant or nursing, talk with your pharmacist or doctor before using this.
Meds to double-check
Moderate interaction found
Before you start, double-check if you take warfarin or other blood thinners (Moderate risk of bleeding), corticosteroids or water pills (Moderate risk of low potassium), digoxin or other heart medications (Moderate risk of toxicity), other stimulant laxatives (Moderate risk of fluid and electrolyte problems), or medications your liver processes via the CYP3A4 pathway (Minor risk of reduced effectiveness).
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
Cascara Sagrada is short-term bowel support for people without the conditions or medications listed in the interaction checker. If you take blood thinners, heart medications, diuretics, or corticosteroids, you need to check with your pharmacist first — the risks are real.
Avoid long-term use and don't use it during pregnancy or while breastfeeding.

Cascara Sagrada 270 mg by Nature's Way Drug Interactions
HelloPharmacist Interaction Report
Cascara Sagrada 270 mg by Nature's Way does interact with several medication types, and the concerns center on how it affects your body's potassium levels and blood clotting.
The most serious risk is with blood thinners like warfarin (Coumadin) — cascara can cause diarrhea, which boosts warfarin's blood-thinning effects and raises your bleeding risk.
Read the full breakdown — every affected drug type, severity by severity
Corticosteroids, water pills (diuretics), and digoxin (a heart medication) all carry Moderate-severity risks because cascara's stimulant laxative effects can lower your potassium, and that matters a lot with digoxin — low potassium can trigger dangerous heart rhythms. If you're taking other stimulant laxatives, cascara can compound fluid and electrolyte losses.
There's also a Minor interaction with certain medications your liver processes (CYP3A4 substrates), as cascara may weaken their effects.
Altogether, these interactions span 735 individual medications. Run your exact prescriptions through the checker below before starting this product.
