Chinese White Peony is generally well tolerated in the short term, but high-quality safety data are limited. The most common side effects when taken by mouth are gastrointestinal: abdominal distension, loss of appetite, diarrhea, nausea, and general stomach discomfort.

In one clinical study, diarrhea occurred in 5% of patients taking a related peony preparation (TGP) at a higher dose for 24 weeks, compared to 1% on placebo. There's also a case report of increased bleeding and bruising in someone on warfarin after switching to a different Chinese herbal product containing peony.

The safety data advise against use during pregnancy—there isn't enough information to consider it safe. For breastfeeding, there also isn't reliable information to know whether it's safe, so avoid it unless you've checked with your doctor or pharmacist.
