Oral comfrey is unsafe. When taken by mouth, comfrey's pyrrolizidine alkaloid constituents can cause serious liver damage — including cirrhosis, hepatic fibrosis, ascites (fluid buildup in the belly), and sinusoidal obstruction syndrome (a blockage of tiny blood vessels in the liver).
These effects can develop gradually and may not show up for months or even up to 2 years. The compound can also damage your lungs, causing pulmonary hypertension.
Topically on unbroken skin, comfrey is generally well tolerated, though skin irritation, rash, itching, and eczema have been reported. Comfrey is not safe in pregnancy or while breastfeeding.
The safety data advises against both because of liver toxicity risks to you and possible harm to your baby or nursing infant.

CYP3A4 enzymes convert pyrrolizidine alkaloids, constituents of comfrey, to toxic metabolites. Some case reports show that enzyme inducers, such as phenobarbital, seem to enhance the toxicity of comfrey.
Hepatotoxic Drugs
Theoretically, comfrey might have additive adverse effects on the liver when used with hepatotoxic drugs.
Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.
