The evidence for ribose's effectiveness is mixed and limited. For athletic performance and McArdle disease, it's rated possibly ineffective.
For restless legs syndrome, muscle soreness, heart failure, and recovery after heart surgery, there isn't enough reliable evidence to rate it either way. In other words, we don't have solid data showing it works for any of these conditions.
How safe is it?
Well-documented data
Ribose is generally well tolerated in the short term — up to about a month in studies — but long-term safety hasn't been well studied, so caution is warranted if you're thinking of using it long-term. The most common side effects are gastrointestinal: diarrhea, nausea, and stomach discomfort.
Headache, low blood sugar, increased appetite, lightheadedness, and mild anxiety have also been reported; lowering the dose has resolved these symptoms in some cases. There isn't enough safety information to recommend ribose during pregnancy or while breastfeeding.
