Saccharomyces boulardii has the most evidence: it's possibly effective for rotaviral diarrhea, traveler's diarrhea, antibiotic-associated diarrhea, necrotizing enterocolitis, C. difficile infection, and H. pylori. Bacillus coagulans is possibly effective for constipation and irritable bowel syndrome (IBS), but the evidence for antibiotic-associated diarrhea is still being studied.
Bacillus subtilis is possibly effective for antibiotic-associated diarrhea, though evidence for other uses like athletic performance, cirrhosis, or eczema remains insufficient. We hold no effectiveness data for Bacillus clausii.
How safe is it?
Well-documented data
These probiotics are generally well tolerated in healthy adults, but several cautions apply. Critically ill patients or those with weakened immune systems should avoid them — rare cases of bloodstream infection from these live organisms have been reported in ICU patients, especially those with central catheters, on broad-spectrum antibiotics, or receiving feeding tubes.
Healthy people rarely face this risk. Side effects are uncommon but may include abdominal cramps, gas, nausea, vomiting, decreased appetite, headache, or diarrhea.
Because safety data are limited, talk with your doctor or pharmacist before starting, particularly if you have a weakened immune system or serious illness. Pregnancy and lactation safety data are not on file for these ingredients — discuss with your healthcare provider.

Saccharomyces boulardii (a live yeast) can theoretically be harmed by antifungal medications in the same way. Additionally, there are rare but serious adverse events to know about: in critically ill patients or those with weakened immune systems, these live organisms have occasionally caused bloodstream infections (fungemia or bacteremia), particularly in ICU settings or with central catheters.
Healthy people with intact immune systems rarely face this risk.

Bacillus coagulans HC
Antibiotic Drugs
Theoretically, taking antibiotics with Bacillus coagulans might decrease the effectiveness of B. coagulans.
B. coagulans preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. coagulans preparations by at least two hours.
Bacillus subtilis HU58
Antibiotic Drugs
Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
Saccharomyces boulardii
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
