The evidence for diindolylmethane isn't established yet. Our data shows insufficient reliable evidence to rate it for benign prostatic hyperplasia (enlarged prostate), breast cancer, colorectal cancer, cervical dysplasia (abnormal cervical cells), or mastalgia (breast pain).
That doesn't mean it doesn't work — it means the science simply isn't there to confirm one way or the other.
How safe is it?
Well-documented data
Diindolylmethane is generally well tolerated in short-term studies, but long-term safety hasn't been studied well. The most common side effects are diarrhea, gas, headache, nausea, rash, and vomiting.
A rare serious side effect — drug rash with eosinophilia and systemic symptoms (DRESS) — has been reported. One case of stroke was reported in a woman taking 200 mg daily alongside other supplements, but she had significant stroke risk factors, so it's unclear if DIM played a role.
During pregnancy, the safety data rates this as likely safe. However, there isn't enough safety data for breastfeeding, so it's best avoided while nursing.

Diindolymethane
Diuretic Drugs
Theoretically, diindolylmethane might increase the risk of hyponatremia if used with sodium-depleting diuretics.
Large doses of diindolylmethane (600 mg daily) have been associated with two cases of asymptomatic hyponatremia in clinical research.
Estrogens
Theoretically, diindolylmethane might increase or decrease the effects of estrogens.
Diindolylmethane might have mild estrogenic or antiestrogenic effects. Theoretically, large amounts of diindolylmethane might interfere with hormone replacement therapy.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, diindolylmethane might lower serum levels of CYP1A2 substrates.
In vitro evidence suggests that diindolylmethane can induce CYP1A2. Theoretically, it might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
