Extrazyme-13 contains 13 active digestive enzymes designed to break down food in your digestive tract. The checked ingredients include bromelain (from pineapple), protease (breaks down protein), lipase (breaks down fat), lactase (breaks down milk sugar), and Bacillus subtilis (a beneficial bacterial strain).
The product also lists amylase and several other enzymes and enzyme blends, though we were unable to verify interactions for some of them. The capsules themselves are vegetarian and filled with maltodextrin as an inactive ingredient.

Strong evidence
The evidence supporting these enzymes is mixed. Lactase is effective for lactose intolerance — if that's your concern, this ingredient delivers.
Bacillus subtilis is possibly effective for antibiotic-associated diarrhea. For the other conditions in our data — such as burns, cancer support, chronic venous insufficiency, and general digestive upset — the evidence we hold is insufficient to rate whether these enzymes actually work.

How safe is it?
Well-documented data
Bromelain is generally well tolerated short-term, but long-term safety data is limited. It may cause diarrhea, flatulence, stomach upset, or headache, and rarely can trigger allergic reactions (including throat tightness or asthma if inhaled).
Protease and Bacillus subtilis are generally well tolerated but may cause digestive upset or, rarely, allergic reactions. Lipase has no reported adverse effects in adults, though serious effects are possible in premature infants.
Lactase is generally well tolerated and likely safe in pregnancy. Bromelain should be avoided in pregnancy and breastfeeding because there isn't enough safety data.
Protease, lipase, and Bacillus subtilis are best avoided in pregnancy unless your doctor advises otherwise, and their safety while breastfeeding is unclear — check with your healthcare provider first.

Extrazyme-13 by LifeTIME contains two ingredients with documented interactions: bromelain and Bacillus subtilis.
The most serious concern is bromelain's moderate-severity interaction with blood thinners and antiplatelet drugs (such as warfarin, clopidogrel, or aspirin), where bromelain may increase bleeding risk by thinning the blood itself. There is also a case report of minor bruising when bromelain was combined with naproxen.

Bromelain also has a minor interaction with tetracycline antibiotics, where it may theoretically increase their absorption, though a human study found no significant effect at typical doses. Bacillus subtilis, a live-organism probiotic ingredient, has a moderate interaction with antibiotic drugs — the antibiotics may kill the beneficial bacteria and reduce its intended effect, so the two should be taken several hours apart.

Bacillus subtilis
Antibiotic Drugs
Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
