How safe is it?
Well-documented data
Creatine and stevia are generally well tolerated in most people. Common side effects from stevia include bloating, dizziness, headache, muscle pain, nausea, and numbness, though most resolve within a week; rare allergic reactions have been reported.
Creatine commonly causes dehydration, diarrhea, stomach upset, muscle cramps, and water retention. Grape extract is generally well tolerated, though some people report stomach pain, diarrhea, dry mouth, indigestion, headache, joint pain, or nausea; anaphylaxis to grape skin is rare.
Polygonum cuspidatum is used in traditional medicine but has limited human safety data; its emodin content may cause digestive upset. Pregnancy and breastfeeding data vary by ingredient: creatine should be avoided in both (safety not established); grape concentrated supplements should be avoided in pregnancy due to limited data, but the food itself is likely safe in pregnancy and we hold insufficient data for breastfeeding; stevia in supplement doses lacks safety data in pregnancy and lactation; and polygonum cuspidatum should be avoided in both due to concerns about uterine and bowel effects and insufficient information.

The most serious concern involves stevia extract, grape, and polygonum cuspidatum (hu zhang), which together affect a broad range of drugs—most critically, stevia may increase lithium levels (a Moderate severity interaction affecting the drug's clearance).

Grape interacts with multiple drug-metabolizing enzymes and blood thinners. It may reduce levels of certain medications broken down by CYP1A2 (like some pain relievers and stimulants), increase levels of drugs metabolized by CYP2D6 and CYP2E1 (including some antidepressants and blood pressure drugs), and may interfere with CYP3A4 substrates (a broad class including statins, immunosuppressants, and many others).
Additionally, grape extracts may increase bleeding risk with blood thinners and antiplatelet drugs, and can reduce absorption of cyclosporine (an immunosuppressant) by up to 30%.
Polygonum cuspidatum (hu zhang) shares similar enzyme interactions—it may increase levels of drugs metabolized by CYP1A2, CYP2C19, CYP2E1, and CYP3A4. It also theoretically increases bleeding risk with anticoagulants and antiplatelet agents, and may interfere with estrogen replacement therapy.
Stevia may lower blood pressure (risk of dizziness or weakness with blood pressure drugs) and theoretically increase low blood sugar risk with diabetes medications. Altogether, these interactions span 1,110 individual medications.

Stevia extract
Lithium
Theoretically, stevia might decrease clearance and increase levels of lithium.
Animal research suggests that stevia extracts might have diuretic activity. Theoretically, increased reabsorption of lithium along with sodium might reduce excretion and increase levels of lithium.
Antidiabetes Drugs
Theoretically, stevia might increase the risk for hypoglycemia when combined with antidiabetes drugs.
Preliminary clinical research in patients with type 2 diabetes suggests that taking a single dose of stevia extract 1000 mg reduces postprandial blood glucose levels when taken with a meal. However, other clinical research in patients with type 1 or type 2 diabetes suggests that taking stevioside 250 mg three times daily does not significantly affect blood glucose levels or glycated hemoglobin (HbA1C) after three months of treatment.
Antihypertensive Drugs
Theoretically, combining stevia or stevia constituents with antihypertensive agents might increase the risk of hypotension.
Stevia extract and stevioside might lower blood pressure in patients with hypertension. However, other clinical research suggests that stevioside does not significantly lower blood pressure in patients with hypertension.
