Does it work?
Moderate evidence
Evidence for Lactobacillus acidophilus shows it is possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis. Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections.
Bifidobacterium longum has insufficient evidence for all conditions listed in our data, including age-related cognitive decline, hay fever, antibiotic-associated diarrhea, anxiety, asthma, and athletic performance. Fructooligosaccharides similarly shows insufficient evidence for osteoporosis, constipation, diabetes, traveler's diarrhea, and high cholesterol.
The evidence we hold does not establish effectiveness for the three probiotic strains we could not check.
How safe is it?
Well-documented data
The checked probiotic bacteria are generally well tolerated in healthy adults and children, though caution is advised if you have a weakened immune system or are seriously ill — talk with your doctor first. The most common side effects are gastrointestinal: abdominal pain, bloating, flatulence, and diarrhea occur uncommonly with probiotics, though they are mild and temporary for most people.
Fructooligosaccharides may cause abdominal pain, bloating, and flatulence, especially at higher doses; studies show these effects are generally mild under 10 grams daily. Very rarely, the live bacteria have been linked to serious infections (bacteremia, sepsis) in critically ill or premature patients; this risk is extremely low in healthy people but should be discussed with your doctor if you are immunocompromised.
For pregnancy, L. acidophilus and B. bifidum are rated possibly safe; B. longum and FOS are rated possibly safe in both pregnancy and lactation, but limited data exist — talk with your healthcare provider about your individual situation.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium longum
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
