What’s inside
Full disclosure
This product contains three active ingredients: vitamin B12, vitamin B6, and metafolin — a form of folate. Vitamin B12 plays a crucial role in energy metabolism, nerve function, and the formation of red blood cells.
Vitamin B6 supports brain development, immune function, and helps your body use amino acids and proteins. Metafolin is a processed form of folate that your body can use directly.
The product also contains plant fiber, cellulose, and water as inactive ingredients.
Does it work?
Strong evidence
Vitamin B12 is effective for treating actual B12 deficiency and two specific genetic conditions (Imerslund-Grasbeck disease) and is likely effective for cyanide poisoning. It's possibly effective for canker sores and postherpetic neuralgia (nerve pain after shingles).
Vitamin B6 is effective for sideroblastic anemia and B6 deficiency, and likely effective for hyperhomocysteinemia (elevated levels of an amino acid linked to heart disease); it's possibly effective for pregnancy-related nausea and vomiting. We don't have effectiveness data on file for metafolin in this product.
How safe is it?
Well-documented data
Vitamin B12 is generally very well tolerated — your body excretes excess amounts in urine, and there's no established upper limit. Serious adverse effects are rare; when they do occur, they're mostly injection-site reactions (this is oral) or rare allergic reactions.
Vitamin B6 is well tolerated at doses below 100 mg daily, but the facts note a caution: at high doses over time, it can damage nerves (sensory neuropathy). Common side effects at normal doses are mild — abdominal pain, headache, nausea — and allergic reactions can occur.
Both B12 and B6 are considered likely safe in pregnancy at recommended amounts and during breastfeeding, though B6 carries a 'possibly unsafe' rating in pregnancy and should only be used under a doctor's guidance for morning sickness. The safety data for metafolin isn't on file.
Meds to double-check
Moderate interaction found
Before taking this product, check with your doctor or pharmacist if you take antihypertensive drugs (blood pressure medications), seizure medications like phenobarbital or phenytoin, amiodarone (a heart rhythm drug), levodopa for Parkinson's disease, or metformin for diabetes. These interactions are rated Moderate (except metformin, which is Minor) and may affect how your medications work or your blood pressure control.
The bottom line
Scorecard at a glanceFully disclosed formula with strong clinical evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
This product may help if you have a confirmed B12 or B6 deficiency, or if your doctor recommends folate supplementation. If you take blood pressure medications, seizure medications, amiodarone, levodopa, or metformin, talk with your doctor or pharmacist before starting — they'll want to monitor you and may adjust your medications.

Folate 5,000 Plus by Pure Encapsulations Drug Interactions
HelloPharmacist Interaction Report
Folate 5,000 Plus by Pure Encapsulations contains vitamin B12 and vitamin B6, both of which have documented interactions with medications.
The most serious concern is with antihypertensive drugs (blood pressure medications), antiseizure medications (phenobarbital and phenytoin), and amiodarone — a heart rhythm medication — all rated as Moderate severity interactions with vitamin B6.
Read the full breakdown — every affected drug type, severity by severity
Vitamin B6 may add to the blood-pressure-lowering effect of antihypertensive medications, potentially lowering your blood pressure more than intended. It can also interfere with how your body metabolizes phenobarbital and phenytoin (seizure medications) at high doses, making those drugs less effective — though it's not clear whether the dose in this product reaches that threshold.
Additionally, vitamin B6 may worsen photosensitivity (sun sensitivity) caused by amiodarone. If you take levodopa for Parkinson's disease without carbidopa, vitamin B6 can reduce its effectiveness, though this interaction does not occur with the combination product Sinemet (levodopa with carbidopa).
Vitamin B12 has a Minor interaction with metformin (Glucophage), the common diabetes medication: metformin can lower B12 absorption over time, potentially contributing to a deficiency with prolonged use. We could not check the interaction data for metafolin, an ingredient in this product.
Altogether, these interactions span 230 individual medications.

For each ingredient in Folate 5,000 Plus with known interactions, here are the types of medications they can affect. Open any type for the detail — or search your exact drug in the checker above.
Vitamin B6
Amiodarone (Cordarone)
Theoretically, vitamin B6 might increase the photosensitivity caused by amiodarone.
Despite initial case reports suggesting that pyridoxine may have a protective effect against amiodarone-induced photosensitivity, preliminary clinical research suggests that pyridoxine may actually exacerbate this adverse effect.
Antihypertensive Drugs
Theoretically, vitamin B6 may have additive effects when used with antihypertensive drugs.
Research in hypertensive rats shows that vitamin B6 can decrease systolic blood pressure. Similarly, clinical research in patients with hypertension shows that taking high doses of vitamin B6 may reduce systolic and diastolic blood pressure, possibly by reducing plasma levels of epinephrine and norepinephrine.
Phenobarbital (Luminal)
High doses of vitamin B6 may reduce the levels and clinical effects of phenobarbital.
Preliminary clinical evidence suggests that vitamin B6 200 mg daily can reduce plasma levels of phenobarbital, possibly by increasing metabolism. It is not known whether lower doses have any effect. Advise people taking phenobarbital to avoid high doses of vitamin B6.
Phenytoin (Dilantin)
High doses of vitamin B6 may reduce the levels and clinical effects of phenytoin.
Preliminary clinical evidence suggests that vitamin B6 200 mg daily can reduce plasma levels of phenytoin, possibly by increasing metabolism. It is not known whether lower doses have any effect. Advise people taking phenytoin to avoid high doses of vitamin B6.
Levodopa
Vitamin B6 may increase the metabolism of levodopa when taken alone, but not when taken in conjunction with carbidopa.
Vitamin B6 (pyridoxine) enhances the metabolism of levodopa, reducing its clinical effects. However, this interaction does not occur when carbidopa is used concurrently with levodopa (Sinemet). Therefore, it is not likely to be a problem in most people.
Vitamin B12
Metformin (Glucophage)
Metformin, a common medication used to manage type 2 diabetes, has been associated with lower vitamin B12 levels in some individuals. Prolonged use of metformin can interfere with the absorption of B12 in the digestive system, potentially leading to a deficiency in this essential vitamin.
