Grass Juice Plus contains 7 ingredients. The active ones are enzymes and plant juices: amylase and lipase (digestive enzymes), plus powdered juices from alfalfa, barley, wheat, kamut, and oats.
The product also contains rice flour as an inactive ingredient (a filler).
Does it work?
Strong evidence
Evidence for this product's effectiveness is mixed. Barley juice and oat juice are both likely effective for high cholesterol and heart disease prevention.
Lipase and alfalfa don't have reliable evidence backing their use for the conditions they're marketed for—the data we hold rates them as having insufficient evidence. The same goes for the other juice powders in terms of what's been documented in our data.
How safe is it?
Well-documented data
Lipase is generally well tolerated, but people with serious digestive conditions should use it only under medical guidance. There's little safety data on lipase in pregnancy or while breastfeeding, so check with your doctor first on both.
Alfalfa is generally well tolerated in food amounts, but the concentrated supplement form has estrogen-like activity and is best avoided during pregnancy. Not enough data exists for safety while breastfeeding.
Barley and oats are safe as foods; as concentrated supplements in pregnancy and breastfeeding, safety is less well studied. All of these ingredients can cause bloating, flatulence, and abdominal discomfort—side effects often seen when you first add fiber-rich supplements.
Barley contains gluten and can trigger allergic reactions in sensitive people.

The most serious concern is alfalfa juice powder, which has a Major interaction with warfarin (Coumadin), a blood thinner. Alfalfa is high in vitamin K, which can reduce how well warfarin works at preventing blood clots.

Alfalfa also has Moderate interactions with several drug types. It may interfere with birth control pills and hormone therapy because it contains plant compounds with estrogen-like activity.
It can theoretically increase the risk of low blood sugar (hypoglycemia) when combined with diabetes medications, and may reduce the effectiveness of drugs that suppress the immune system. There's also a theoretical risk of increased sun sensitivity if you're taking photosensitizing medications.
Barley juice powder has a Moderate interaction with triclabendazole (Egaten), a medication used for certain parasitic infections—barley may reduce how well it works. Oat juice powder may increase the risk of low blood sugar when taken with diabetes drugs or insulin, so blood glucose monitoring is important if you're on those medications.

Alfalfa Juice, Powder
Warfarin (Coumadin)
Theoretically, alfalfa might reduce the anticoagulant activity of warfarin.
Alfalfa contains a large amount of vitamin K. This could theoretically interfere with the activity of warfarin.
Antidiabetes Drugs
Theoretically, alfalfa might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that alfalfa decreases blood sugar in diabetic mice. Also, in one case report, a diabetic patient experienced hypoglycemia after consuming alfalfa extract. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Contraceptive Drugs
Theoretically, alfalfa might interfere with the activity of contraceptive drugs.
Alfalfa contains coumestrol, a phytoestrogen, and isoflavonoids, which have estrogenic effects.
Estrogens
Theoretically, alfalfa might interfere with hormone therapy.

Immunosuppressants
Theoretically, alfalfa might decrease the efficacy of immunosuppressive therapy.
In vitro research and human case reports suggest that alfalfa may have immunostimulant effects.
Photosensitizing Drugs
Theoretically, concomitant use of alfalfa with photosensitizing drugs might have additive effects.
Animal research suggests that excessive doses of alfalfa may increase photosensitivity, possibly due to its chlorophyll content. It is unclear if this effect would be clinically relevant in humans.
Wheat juice powder
Antidiabetes Drugs
Theoretically, taking wheatgrass with antidiabetes drugs might lower blood glucose levels and increase the risk of hypoglycemia.
Animal research shows that taking wheatgrass stimulates the release of insulin from beta-cells and lowers blood glucose.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, wheatgrass might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that wheatgrass induces CYP1A2 enzymes.
Oat juice powder
Antidiabetes Drugs
Theoretically, oats may have additive effects with antidiabetic agents and might increase the risk of hypoglycemia.
Consuming oats can decrease blood glucose in patients with diabetes. In those who require insulin, taking oats 100 grams daily for 2 days reduces the insulin dose required to achieve metabolic control.
Insulin
Concomitant use of oats and insulin might increase the risk of hypoglycemia.
In patients with insulin-dependent type 2 diabetes, taking oats 100 grams daily for 2 days reduces the insulin dose required to achieve metabolic control.
Barley juice powder
Triclabendazole (Egaten)
Theoretically, barley might decrease the clinical effects of triclabendazole.
Animal research suggests that a diet supplemented with barley can reduce the bioavailability of triclabendazole when taken concomitantly. This effect has not been shown in humans.
