HelloPharmacist Scorecard of Herbal Prostate Combo by Swanson
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains 3 active ingredients. Pygeum africanum is an African tree bark extract used in formulas for men's prostate health.
Saw palmetto is a berry extract also targeted at prostate and urinary symptoms. Stinging nettle—the aerial parts of the plant—rounds out the blend, traditionally used for general wellness in prostate-related formulas.
The capsules also contain gelatin and magnesium stearate as inactive ingredients.
Does it work?
Strong evidence
Pygeum africanum is likely effective for benign prostatic hyperplasia (BPH), the noncancerous prostate enlargement that causes urinary symptoms. Saw palmetto, by contrast, is possibly ineffective for BPH, though it may be possibly effective if you've had a prostate procedure called TURP.
For stinging nettle, the evidence in our data is insufficient to rate it for BPH or most other conditions, though it is possibly effective for diabetes. None of the three ingredients have strong evidence for other uses listed in this product's data.
How safe is it?
Well-documented data
Pygeum and saw palmetto are generally well tolerated short-term in adults, but long-term safety data are limited for pygeum, and saw palmetto has infrequent but documented side effects—mostly mild and reversible, including nausea, diarrhea, headache, and dizziness. Rare case reports describe serious heart rhythm issues with saw palmetto and a fixed drug eruption (blistering rash).
Stinging nettle is generally well tolerated; most adverse effects reported are constipation or diarrhea. Topical contact with fresh stinging nettle plant causes itching and rash.
Do not use this product if you are pregnant or breastfeeding. Pygeum, saw palmetto, and stinging nettle all lack adequate safety data in pregnancy, and saw palmetto is rated likely unsafe.
Stinging nettle also raises traditional concerns about effects on the uterus.
Meds to double-check
Moderate interaction found
Double-check this product if you take anticoagulants or antiplatelet drugs (blood thinners like warfarin), hormonal contraceptives or estrogen therapy, diabetes medications, lithium, or diuretics (water pills). These are the medication types with documented Moderate-severity interactions.
No interactions are documented for pygeum africanum among our data.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This product is intended for men's prostate health, and pygeum has solid evidence for BPH. If you take any blood thinners, hormonal contraceptives, diabetes medications, diuretics, or lithium, you need to check this product with your doctor or pharmacist before starting—the interactions are real and could affect how your medications work.
Avoid it entirely during pregnancy or while breastfeeding.

Herbal Prostate Combo by Swanson Drug Interactions
HelloPharmacist Interaction Report
Herbal Prostate Combo by Swanson contains three ingredients, and two of them interact with medications.
Saw palmetto carries the most serious concerns: it may reduce the effectiveness of hormonal contraceptives and estrogens, and it may increase bleeding risk if you take anticoagulants or antiplatelet drugs (blood thinners)—all of these are rated Moderate severity.
Read the full breakdown — every affected drug type, severity by severity
Stinging nettle also has four Moderate-severity interactions to watch. It may lower blood sugar (glucose) too much if you take diabetes medications, and it may increase lithium levels in your blood if you take lithium for mood disorders—potentially requiring a dose adjustment.
It also has significant vitamin K content, which could interfere with warfarin (Coumadin) and similar blood thinners. Additionally, combining it with water pills (diuretics) might amplify their effect.
Pygeum africanum, the third ingredient, has no known interactions documented in our data. Altogether, these interactions span 335 individual medications.
