The probiotics in this blend have some evidence behind them. Lactobacillus acidophilus is rated possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Fungal lactase is rated effective for lactose intolerance — straightforward enzyme replacement if that's one reason you're considering this blend. Common side effects are mild: papain may trigger allergic reactions in sensitive people (itchy watery eyes, runny nose, sneezing, abdominal cramps, diarrhea); bromelain commonly causes diarrhea, flatulence, gastric upset, and headache; and the probiotics may cause gastrointestinal discomfort, bloating, or flatulence, though these are uncommon.
At high doses, papain carries rare risks of severe gastritis or esophageal perforation. Papain is rated possibly unsafe in pregnancy due to safety concerns with concentrated forms and unripe papaya products, so talk with your doctor if you're pregnant.
Bromelain is rated unsafe in pregnancy and should be avoided while breastfeeding. Lactobacillus acidophilus is rated possibly safe in pregnancy, but data on it and the other ingredients during breastfeeding are limited or absent — check with your healthcare provider before use if you're nursing.

The most serious is a Moderate interaction between papain and warfarin (a blood thinner), where papain may increase warfarin's effects and side effects — one case report documented a patient's blood-thinning level (INR) rise to 7.4 after taking a papain-containing supplement.

Bromelain interacts with two drug types. It may increase bleeding risk with anticoagulants or blood thinners (Moderate severity) and theoretically might raise tetracycline antibiotic levels (Minor severity), though a human study found no actual difference in tetracycline blood levels.
Lactobacillus acidophilus could theoretically reduce its own effectiveness if taken at the same time as antibiotics, since antibiotics kill the live organisms in the probiotic (Moderate severity). The product also contains Lactobacillus plantarum, Alpha-Galactosidase, and Lactobacillus casei, which we could not check for interactions, as well as Fungal Protease, Fungal Lipase, and Fungal Lactase, which were checked and show no documented interactions.

Lactobacillus acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
