HelloPharmacist Scorecard of Indole-3-Carbinol by Nature's Sunshine
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
This product contains 2 active ingredients: indole-3-carbinol and broccoli, delivered in a proprietary blend. Indole-3-carbinol is a compound extracted from cruciferous vegetables like broccoli and cabbage; broccoli here appears as a concentrated ingredient as well.
The capsule itself and the other materials inside — vegetable magnesium stearate and silicon dioxide — are inactive ingredients that help hold the formula together.
Does it work?
Not established
The evidence we hold doesn't establish that this product works for the conditions listed. For indole-3-carbinol, the data shows insufficient evidence to rate it for breast cancer, colorectal cancer, cervical dysplasia, liver disease, and fibromyalgia.
Broccoli has possibly effective evidence for colorectal cancer, but for breast cancer, bladder cancer, gastric cancer, and fibromyalgia, the evidence is also insufficient. In short, we don't have solid data showing this supplement prevents or treats any of these conditions.
How safe is it?
Well-documented data
Indole-3-carbinol is generally well tolerated when taken orally, though a small number of people have reported diarrhea, nausea, rash, tremor, and unsteadiness — especially at higher doses. One case showed that imbalance and tremor resolved when the dose was cut in half.
Broccoli as food is safe for most people, but the safety data on concentrated broccoli supplements is less established. The safety notes on file advise caution with concentrated supplements and recommend avoiding them unless your doctor approves.
Regarding pregnancy: there isn't enough data to know whether this product is safe in pregnancy — talk with your doctor or pharmacist for personalized advice. The data advises against using indole-3-carbinol in concentrated supplement form while breastfeeding.
Meds to double-check
Moderate interaction found
Before taking this product, check with your pharmacist if you're on blood thinners or antiplatelet drugs — indole-3-carbinol may increase bleeding risk. Also double-check any medications metabolized by your liver enzymes CYP1A2 or CYP2A6, as both ingredients in this product can lower their blood levels and reduce how well they work.
If you take estrogen therapy or hormonal birth control, indole-3-carbinol may interfere with its effectiveness.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
This product combines two ingredients that both speed up how your liver breaks down medications, which can make those drugs less effective — and one of them may also raise bleeding risk if you're on blood thinners or interfere with hormone therapy. If you take any prescription medication, run it through the checker below before you start.
A conversation with your pharmacist is especially important before taking this product.

Indole-3-Carbinol by Nature's Sunshine Drug Interactions
HelloPharmacist Interaction Report
Indole-3-Carbinol by Nature's Sunshine contains two active ingredients — indole-3-carbinol itself and broccoli — and both interact with medications.
The most serious interaction is a Moderate-severity effect on blood thinners (anticoagulants and antiplatelet drugs): indole-3-carbinol may increase bleeding risk by interfering with how platelets clump together.
Read the full breakdown — every affected drug type, severity by severity
Both ingredients also affect how your liver processes certain medications through an enzyme called CYP1A2. Indole-3-carbinol may speed up that breakdown and lower the blood levels of affected drugs; broccoli does the same, and research shows it can increase this enzyme activity by 10% to 200% depending on the dose.
Additionally, broccoli affects another liver enzyme, CYP2A6, potentially reducing the effectiveness of drugs it processes — human studies show increases in that enzyme activity of 135% to 550%.
Indole-3-carbinol also carries a Moderate interaction with estrogen therapy (hormonal birth control or hormone replacement): it may interfere with how well those hormones work. Altogether, these interactions span 295 individual medications.

Indole-3-carbinol might interfere with the effects of estrogen therapy.
Preliminary clinical and in vitro evidence shows that indole-3-carbinol has antiestrogenic activity.
Broccoli
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, broccoli might reduce the levels and effects of drugs metabolized by CYP1A2.
Pharmacokinetic research in humans shows that eating 500 grams of fresh broccoli daily for 6-12 days can increase CYP1A2 activity by 10% to 200%. Induction of CYP1A2 activity by broccoli is attributed to its glucosinolate constituents.
Cytochrome P450 2A6 (Cyp2A6) Substrates
Theoretically, broccoli might reduce the levels and effects of drugs metabolized by CYP2A6.
Pharmacokinetic research in humans shows that eating 500 grams of broccoli daily for 6 days increases CYP2A6 activity by 135% to 550%. Induction of CYP2A6 activity is attributed to its glucosinolate constituents.
