Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: ipriflavone, a compound structurally similar to a plant estrogen. The capsule itself is made from bovine gelatin, with cellulose and magnesium stearate as inactive ingredients to hold it together and aid processing.
Does it work?
Strong evidence
Ipriflavone is rated Likely Effective for osteoporosis (bone loss) and Paget disease (a bone disorder). The evidence supports its use for these conditions, though you'd want to discuss with your doctor whether it's the right choice for your situation.
How safe is it?
Well-documented data
Ipriflavone is generally well tolerated when taken by mouth, though some people report stomach pain, diarrhea, or dizziness. The bigger concern: long-term safety data is limited, and there's a risk of low white blood cell counts (lymphocytopenia) if you take it for at least 6 months or longer.
Because of possible estrogen-like effects and insufficient safety information, ipriflavone is not recommended during pregnancy or breastfeeding. If you're pregnant, planning to become pregnant, or breastfeeding, talk with your doctor or pharmacist before taking it.
Meds to double-check
Moderate interaction found
If you take theophylline, any blood thinners, muscle relaxants, antidepressants, or immunosuppressant drugs, you need to check with your doctor or pharmacist before adding ipriflavone. Given the scale of potential interactions (affecting over 300 medications), it's wise to review your entire medication list before starting.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Ipriflavone may be worth considering if you have osteoporosis or Paget disease and your doctor agrees—but it interacts with a large number of medications, especially theophylline, blood thinners, and immune-suppressing drugs. Before starting, run through your medication list with your doctor or pharmacist.
It's also not suitable during pregnancy or breastfeeding.

Ipriflavone 300 mg by NOW Drug Interactions
HelloPharmacist Interaction Report
NOW's Ipriflavone 300 mg interacts with medications through its single active ingredient.
The most serious concern is with theophylline (a medication used for breathing problems): ipriflavone can raise theophylline levels in your blood to potentially unsafe amounts by slowing how your body clears it.
Read the full breakdown — every affected drug type, severity by severity
Ipriflavone also affects how your liver metabolizes a broad range of other drugs. It inhibits two liver enzymes (CYP1A2 and CYP2C9) that break down many common medications—including muscle relaxants, certain antidepressants, and some blood thinners—which could raise their levels and increase the risk of side effects.
Altogether, these interactions span 391 individual medications.
There's also a Moderate-severity concern with immunosuppressant drugs (medications that weaken your immune system on purpose, such as those used after organ transplant). Ipriflavone may add to that effect and has been linked to low white blood cell counts in some people taking it for 6 months or longer.
Before you start ipriflavone, check your exact medications with your doctor or pharmacist—especially if you take theophylline, blood thinners, muscle relaxants, antidepressants, or any drug that suppresses immunity.

Ipriflavone
Cytochrome P450 1A2 (Cyp1A2) Substrates
Ipriflavone is thought to competitively inhibit cytochrome P450 1A2 (CYP1A2). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.
Immunosuppressants
Concurrent use of ipriflavone and immunosuppressant drugs might have additive effects on inhibiting immune function. Some patients taking ipriflavone experience subclinical lymphocytopenia; avoid concurrent use. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).
Theophylline
Concomitant use of theophylline and ipriflavone can increase serum theophylline levels. Increased theophylline levels have occurred in two cases after patients started taking ipriflavone. Ipriflavone is thought to decrease theophylline metabolism through competitive inhibition of the cytochrome P450 1A2 (CYP1A2) enzyme.
