The liquid also includes purified water, glycerin, citric acid, sodium benzoate, and natural flavor as inactive ingredients.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency and possibly effective for high cholesterol (hyperlipidemia), congestive heart failure, and angina. Pantothenic acid is effective for pantothenic acid deficiency.
For other uses pantothenic acid has been studied for—like hay fever, alcoholism, and hair loss—the evidence isn't strong enough to rate it in our data.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses, but high doses can cause stomach upset and a fishy body odor. Common side effects include abdominal cramps, diarrhea, heartburn, nausea, and reduced appetite.
Rarely, seizures have been reported. Pantothenic acid taken by mouth is generally well tolerated, though diarrhea can occur.
There isn't enough reliable safety data for L-carnitine during pregnancy; caution is advised. Pantothenic acid is likely safe in pregnancy and breastfeeding at normal dietary amounts and prenatal-vitamin doses, though high-dose supplements in pregnancy warrant a doctor's approval.
For breastfeeding, safety of L-carnitine isn't well studied.

L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
