Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: L-carnitine, an amino acid your body naturally makes and uses to turn fat into energy. The capsule also contains inactive ingredients (gelatin, calcium carbonate, starch, silica, and vegetable lubricant) that serve as fillers and binders.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency. For other uses — high cholesterol (hyperlipidemia), heart failure (congestive heart failure), chest pain (angina) — the evidence is possibly effective, meaning there's some promising research but it's not yet definitive.
The data we hold doesn't establish effectiveness for other purposes.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. At higher doses, you may experience stomach upset, including nausea, diarrhea, heartburn, abdominal cramping or pain, and a fish-like body odor.
Seizures are rare but have been reported. There isn't enough reliable safety data for this product in general, so use only under medical supervision.
For pregnancy, the safety data isn't on file for this product. For breastfeeding, the evidence rates it as possibly safe, though it's not well studied — talk with your doctor or pharmacist about whether it's right for you.
High doses can also cause stomach upset and that characteristic fishy odor.
Meds to double-check
Moderate interaction found
Before taking this product, double-check with your doctor or pharmacist if you take any blood thinners (warfarin or acenocoumarol) or thyroid hormone replacement — L-carnitine can increase bleeding risk or reduce how well your thyroid medication works. No interactions are documented for the other 16 medications affected in our data.
The bottom line
Scorecard at a glanceFully disclosed formula with strong clinical evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
L-Carnitine 500 is most clearly useful if you have a documented L-carnitine deficiency. If you're taking a blood thinner or thyroid medication, you'll need to check with your doctor or pharmacist first — this product can interfere with both.
For other uses like heart health or cholesterol, the research is promising but not yet conclusive.

L-Carnitine 500 by Progressive Professional from Progressive Laboratories Drug Interactions
HelloPharmacist Interaction Report
L-Carnitine 500 from Progressive Professional interacts with blood thinners (anticoagulants) and thyroid hormone replacement.
The most serious concern is with warfarin (Coumadin) and acenocoumarol (Sintrom), both blood thinners — L-carnitine may increase their anticoagulant effects, meaning your blood could thin more than intended, raising bleeding risk. There are case reports of elevated INR (a measure of blood thinning) when L-carnitine was used alongside acenocoumarol.
Read the full breakdown — every affected drug type, severity by severity
L-carnitine also appears to work against thyroid hormone at the cellular level, potentially decreasing how well thyroid replacement medication works. If you take thyroid hormone for hypothyroidism, this product could reduce its effectiveness.
Altogether, these interactions span 19 individual medications. Before starting L-carnitine, check with your doctor or pharmacist, especially if you take any blood thinner or thyroid medication.

For each ingredient in L-Carnitine 500 with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
