L-carnitine is effective for treating L-carnitine deficiency. The evidence is less established for other uses: it's rated possibly effective for high cholesterol (hyperlipidemia), congestive heart failure (CHF), and angina, meaning the science suggests it may help, but the evidence isn't conclusive yet.

L-carnitine is generally well tolerated at typical doses. At higher doses, it can cause stomach upset — nausea, diarrhea, abdominal cramps, heartburn, and reduced appetite have been reported in trials.
A fishy body odor is also a known side effect. Rarely, seizures have been reported, though this is uncommon.
Not enough reliable safety data exists for L-carnitine outside medical supervision, so talk with your pharmacist or doctor before starting if you have any health concerns. Safety while breastfeeding hasn't been well studied, so caution is advised if you're nursing.

L-carnitine may increase how strongly blood thinners work. This is a Moderate-severity concern with warfarin (Coumadin) and acenocoumarol (Sintrom), a shorter-acting blood thinner.
The effect is theoretical with warfarin, but there are at least two case reports of elevated INR (a measure of blood thinning) when L-carnitine was taken alongside acenocoumarol. If you take either of these, your doctor may need to monitor your blood clotting more closely.
L-carnitine may also reduce how well thyroid hormone replacement works. It appears to block thyroid hormone from entering cells properly, which could weaken the effect of your medication if you take it for an underactive thyroid (hypothyroidism).

L-Carnitine L-Tartrate
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
