Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This supplement contains one active ingredient: nattokinase, an enzyme derived from fermented soy (natto). The capsule also contains inactive ingredients — gelatin, microcrystalline cellulose, magnesium stearate, and maltodextrin — which are fillers and binders used to hold the capsule together.
Does it work?
Not established
The evidence for nattokinase isn't established yet in the data we hold. The product has been studied for angina, atherosclerosis, cancer, cardiovascular disease, and chronic fatigue syndrome, but the research so far is insufficient to say whether it actually works for any of these.
How safe is it?
Well-documented data
Nattokinase is generally tolerated when taken short-term, but because it thins the blood, it's not a good fit if you have a bleeding disorder or high bleeding risk. Rare cases of bleeding — including serious brain bleeding — have been reported, especially when combined with blood thinners.
During pregnancy, there isn't enough safety data, so it's best avoided. The same goes for breastfeeding — safety is unknown, so talk with your doctor or pharmacist about whether it's right for you.
Additionally, if you're sensitive to natto (fermented soy), you could have an allergic reaction, ranging from throat or larynx swelling to anaphylaxis in rare cases.
Meds to double-check
Moderate interaction found
Before taking this product, double-check your medications with your doctor or pharmacist, especially blood thinners (anticoagulants and antiplatelets like aspirin, warfarin, or clopidogrel) and blood pressure medications. Both carry Moderate-severity interaction risks with nattokinase.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
Nattokinase is for people interested in cardiovascular support, but the evidence for its effectiveness isn't clear yet. The main caution is its blood-thinning effect — if you take any blood thinner or blood pressure medication, or if you have a bleeding disorder, check with your doctor or pharmacist before you start.
Skip it during pregnancy and breastfeeding.

Nattokinase 100 mg by Swanson Drug Interactions
HelloPharmacist Interaction Report
Swanson's Nattokinase 100 mg interacts with blood thinners (anticoagulants/antiplatelets) and blood pressure medications through its nattokinase content.
The most serious concern is bleeding risk: nattokinase has blood-thinning effects, and there's a documented case of brain bleeding (intracranial hemorrhage) in a patient who combined nattokinase with aspirin. If you take any blood thinner — including aspirin, warfarin, clopidogrel, or other anticoagulants — you'll want to check with your doctor or pharmacist before starting this.
Read the full breakdown — every affected drug type, severity by severity
Nattokinase may also lower blood pressure. If you're on blood pressure medications (like ACE inhibitors, beta-blockers, or calcium channel blockers), combining this supplement could theoretically increase the risk of blood pressure dropping too low.
Altogether, these interactions span 289 individual medications. Use the medication checker on this page to look up your exact prescriptions before you start.
