Nattokinase is generally tolerated when taken short-term, but because it thins the blood, it's not a good fit if you have a bleeding disorder or high bleeding risk. Rare cases of bleeding — including serious brain bleeding — have been reported, especially when combined with blood thinners.

During pregnancy, there isn't enough safety data, so it's best avoided. The same goes for breastfeeding — safety is unknown, so talk with your doctor or pharmacist about whether it's right for you.
