Hydroxycitric acid (HCA) is the active compound extracted from garcinia, a tropical fruit; manufacturers market it in weight-loss and metabolic support formulas. The capsule also contains gelatin, vegetable stearate, and magnesium silicate as inactive ingredients (binders and flow agents).
Does it work?
Moderate evidence
The evidence for these ingredients is mixed. Chromium is rated likely effective for chromium deficiency and possibly effective for diabetes — meaning some research supports benefit for blood sugar control in certain people.
For prediabetes and schizophrenia, the evidence rates it possibly ineffective. For hydroxycitric acid (HCA), the data we hold shows insufficient reliable evidence across all measured uses — athletic performance, liver health, cholesterol, and obesity.

How safe is it?
Well-documented data
Chromium is generally well tolerated at typical supplement doses. The most common side effects are gastrointestinal irritation, headaches, insomnia, irritability, and mood changes.
Rare but serious effects include kidney and liver damage, muscle breakdown (rhabdomyolysis), and low platelet counts. Skin rashes and hives have also been reported, especially in people with chromium contact allergy.
High or long-term doses carry unknown risks, so avoid extra chromium unless your doctor recommends it. Hydroxycitric acid (HCA) is generally well tolerated short-term in clinical studies.
Common side effects are diarrhea, stomach discomfort, headache, and nausea. Rare serious effects include liver injury, liver failure, mania (especially in people with bipolar disorder), and pancreatitis.
One case of severe heart inflammation leading to heart failure has been reported. During pregnancy, not enough safety data exists — the facts advise against use.
The same applies to breastfeeding.

Blood thinners and anti-platelet drugs (warfarin, clopidogrel, aspirin) interact with HCA, raising bleeding risk. If you take levothyroxine (thyroid hormone), chromium may reduce how much you absorb.
Drugs that can harm the liver may have additive risk with HCA.

The most serious interaction involves hydroxycitric acid with serotonergic drugs (antidepressants and similar medications) — theoretically increasing the risk of serotonin syndrome, a dangerous condition marked by agitation, confusion, rapid heart rate, and high blood pressure. One patient reported serotonin syndrome after combining garcinia extract with the antidepressant escitalopram.

Chromium interacts with blood sugar medications. It may have additive effects with diabetes drugs and insulin, raising the theoretical risk of low blood sugar (hypoglycemia).
Research shows chromium can lower blood glucose and increase insulin sensitivity. Chromium may also decrease how much levothyroxine (thyroid hormone) your body absorbs — one study found a 17% drop in blood levels when chromium picolinate 1000 mcg was taken with levothyroxine 1 mg.
NSAIDs and aspirin may increase how much chromium your body absorbs, though this is a minor interaction.
Hydroxycitric acid may theoretically increase bleeding risk with blood thinners (anticoagulants) and anti-platelet drugs like warfarin or clopidogrel. It also theoretically raises the risk of low blood sugar when combined with diabetes medications — though this effect has been seen in animals, not yet confirmed in humans.
Concomitant use with other drugs that can damage the liver may increase that risk.

Garcinia
Anticoagulant/Antiplatelet Drugs
Theoretically, hydroxycitric acid (HCA), the main active ingredient in garcinia, might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
HCA inhibits platelet aggregation in vitro. The inhibitory effect seems to be greater in platelets extracted from diabetic subjects than non-diabetic subjects.
Antidiabetes Drugs
Theoretically, hydroxycitric acid (HCA), the main active ingredient in garcinia, might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia.
HCA reduces fasting and postprandial blood glucose levels in animal models, theoretically by delaying glucose absorption. This effect has not been reported in humans.
Hepatotoxic Drugs
Theoretically, concomitant use with other potentially hepatotoxic drugs might increase the risk of developing liver damage.
There have been reports of acute hepatitis with elevated liver enzymes associated with garcinia, when taken alone or in combination with other ingredients. Case reports collected from the Drug Induced Liver Injury Network suggest this risk may be greater in people who carry the HLA B*35:01 allele.
Serotonergic Drugs
Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.
Chromium
Antidiabetes Drugs
Theoretically, chromium may have additive effects with antidiabetic agents and increase the risk of hypoglycemia.
Some research shows that taking chromium might lower blood glucose levels, especially in patients with poorly controlled type 2 diabetes.
Insulin
Theoretically, concomitant use of chromium and insulin might increase the risk of hypoglycemia.
In clinical research, chromium has been shown to increase insulin sensitivity,
Levothyroxine (Synthroid, Others)
Chromium might bind levothyroxine in the intestinal tract and decrease levothyroxine absorption.
Clinical research in healthy volunteers shows that taking chromium picolinate 1000 mcg with levothyroxine 1 mg decreases serum levels of levothyroxine by 17% when compared to taking levothyroxine alone. Advise patients to take levothyroxine at least 30 minutes before or 3-4 hours after taking chromium.
Aspirin
Theoretically, aspirin might increase chromium absorption.
Animal research suggests that aspirin may increase chromium absorption and chromium levels in the blood.
Nonsteroidal Anti-Inflammatory Drugs (Nsaids)
NSAIDs might increase chromium levels in the body.
Drugs that are prostaglandin inhibitors, such as NSAIDs, seem to increase chromium absorption and retention.
