In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece worth knowing about this combo cough and cold medicine: one of its three ingredients, dextromethorphan (the cough suppressant), nudges serotonin levels in the body. Garcinia has been loosely linked to serotonin-related effects too. Put them together and, in theory, you could raise the small chance of too much serotonin activity. Signs to watch for include agitation, a racing heart, sweating, shivering, muscle twitching, or feeling confused.
I'll be honest with you, this is built on very thin evidence (a couple of case reports), and the bigger concern with this product is usually the acetaminophen and not overdoing it. Still, if you take other antidepressants or serotonin-affecting meds, check with your pharmacist before pairing these. Don't stop anything on your own.

HEPATOTOXIC DRUGS

There have been reports of acute hepatitis with elevated liver enzymes associated with garcinia, when taken alone or in combination with other ingredients. Case reports collected from the Drug Induced Liver Injury Network suggest this risk may be greater in people who carry the HLA B*35:01 allele.

Here's the thing about this combo product: it contains acetaminophen (the same active ingredient in Tylenol), and acetaminophen is already known to be hard on the liver when taken in high amounts or over a long stretch. Garcinia has its own track record of rare liver trouble, with case reports of hepatitis and elevated liver enzymes. So the worry here is mostly about stacking two things that can both stress the liver.
To be clear, this is a theoretical concern based on how each one behaves, not a proven dangerous reaction. There's no effect on how the cold and cough symptoms get treated. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and don't go over the labeled acetaminophen dose. If you have any liver issues or drink alcohol regularly, check with your pharmacist before pairing these.
