Okra Pepsin E3 contains 12 ingredients in a proprietary blend, including carrot, bovine orchic extract, pepsin (a digestive enzyme), alfalfa, Spanish moss, carbamide, sunflower, pea, buckwheat, alginic acid, okra, and fat soluble extract. The product also contains inactive ingredients—gelatin capsule, cellulose, water, calcium stearate, and vitamin E—that serve as binders, fillers, and preservatives.
Pepsin is included as a protease, an enzyme that helps break down proteins during digestion.

The evidence we hold does not establish that Okra Pepsin E3 is effective for any condition. Carrot is possibly effective for vitamin A deficiency, but has insufficient evidence for breast cancer, colorectal cancer, fibromyalgia, and prostate cancer.
Alfalfa, buckwheat, and okra all carry insufficient evidence ratings for the conditions they're studied in—including diabetes control, cholesterol, and various other uses. How safe is it?
Well-documented data
Carrot is generally well tolerated when eaten as food; as a concentrated supplement, it is less studied. Some people are allergic to carrot, with symptoms ranging from hives and swelling to respiratory or digestive reactions.
Alfalfa is well tolerated in small food amounts but may cause abdominal discomfort, diarrhea, gas, or rarely a lupus-like syndrome with chronic use. Buckwheat and pepsin are also generally well tolerated, though allergic reactions—including asthma and anaphylaxis—can occur.
Bovine orchic extract carries particular concern: it comes from raw bovine testes from slaughterhouses and may be contaminated with diseased tissue; there is no human safety data. Alginic acid and okra have limited human safety data in supplement form.

Alginic Acid
Oral Drugs
Theoretically, algin can alter the absorption of oral drugs. Laboratory research shows that algin, when consumed as part of the diet or used as a thickening agent in infant formula, reduces the absorption of certain minerals. Also, algin has been used as a tablet binder and disintegrant to enhance drug bioavailability. To avoid changes in absorption, take algin 30-60 minutes after oral medications.
Alfalfa
Warfarin (Coumadin)
Theoretically, alfalfa might reduce the anticoagulant activity of warfarin.
Alfalfa contains a large amount of vitamin K. This could theoretically interfere with the activity of warfarin.
Antidiabetes Drugs
Theoretically, alfalfa might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that alfalfa decreases blood sugar in diabetic mice. Also, in one case report, a diabetic patient experienced hypoglycemia after consuming alfalfa extract. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Contraceptive Drugs
Theoretically, alfalfa might interfere with the activity of contraceptive drugs.
Alfalfa contains coumestrol, a phytoestrogen, and isoflavonoids, which have estrogenic effects.
Estrogens
Theoretically, alfalfa might interfere with hormone therapy.

Immunosuppressants
Theoretically, alfalfa might decrease the efficacy of immunosuppressive therapy.
In vitro research and human case reports suggest that alfalfa may have immunostimulant effects.
Photosensitizing Drugs
Theoretically, concomitant use of alfalfa with photosensitizing drugs might have additive effects.
Animal research suggests that excessive doses of alfalfa may increase photosensitivity, possibly due to its chlorophyll content. It is unclear if this effect would be clinically relevant in humans.
Okra
Antidiabetes Drugs
Theoretically, concomitant use of ambrette with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research shows that myricetin, a constituent of ambrette, can lower blood glucose levels.
