Andrographis is generally well tolerated when used short-term, but long-term safety and product quality aren't well established — so don't assume it's proven safe over months or years. The most common side effects are stomach discomfort, taste changes, diarrhea, dizziness, fatigue, headache, nausea, vomiting, itching, rash, and hives.

These become more likely at higher doses (5–10 mg/kg of andrographolide) or beyond 14 days of use. Rare but serious side effects include severe allergic reactions and anaphylaxis.

Skin reactions are frequently reported and can include Stevens-Johnson syndrome (a serious blistering condition). During pregnancy, the safety data advises against use because of theoretical and animal-study concerns about reproductive effects.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
