Royal jelly is generally well tolerated by most people, but it carries a serious risk if you have a history of asthma or allergies (atopy). In sensitive people, it can trigger itching, hives, eczema, swelling of the eyelids and face, breathing problems, or even anaphylaxis and death — these reactions are rare but documented.

Skin irritation and contact dermatitis can occur if applied topically. There's also one case report of severe bleeding in the colon after royal jelly ingestion.
